Welcome to the 2017/2018 School Year!
Dear Breakwater Families,
We're catching up with the times! In an effort to provide a more efficient, streamlined and "green" approach to our yearly information request, we are moving to digital delivery! Please find links to forms necessary for the 2017/2018 school year below. Please take the time to fill out all forms completely and submit them by Wednesday, August 16. If you have difficulty with the online forms or would simply prefer paper copies, please contact me.
It is important that we receive these documents by the deadline for licensing and legal requirements, communication purposes (including newsletters and classroom announcements), your child’s ability to participate in off-campus activities, and in order to complete our 2017/2018 Family Handbook and Directory. Please note that a separate emergency, permissions and immunization form must be completed for each child. A link to 2017/2018 class placement list appears at the bottom of the page.

For your privacy, the class list page is password protected. Currently enrolled families please see our recent email (date: July 30/subject line: "School Forms!") for password, or contact the main office.
